Security forms are required to create security classes and operator ids in the Student Information System (SIS). The files linked below use Excel spreadsheets or Word documents. Complete the forms on your workstation and save them to a file. Email the file to the VCCS Information Security Officer. Only completed forms that are emailed will be accepted.

Word is needed to use the forms below and Excel is needed to view the menu items in the Creating SIS Security Classes Form.
